Welcome to your first Nurse Narrative shift. Your patient is Robert Thomas, 68 years old, in room 226 with an acute exacerbation of congestive heart failure. During this shift, Robert has a change in his breathing. Your job is to turn everything that happened into clear, professional nursing documentation. First, you'll meet your patient and hear what brought you into the room. Then you'll rescue a messy chart by deciding what should stay, what needs better documentation, and what doesn't belong in this focused narrative. After that, you'll organize the important events into a clear clinical story. Remember: good documentation tells the next nurse what happened, what you found, what was done, and how the patient responded. When you're ready, let's begin.
